These terms & conditions are written by Jennifer Wright, owner of RISE: Rehab & Holistic Therapies, under the website; https://www.riseholisticrehab.co.uk. Jenny is a fully qualified and registered Occupational Therapist; HCPC number OT72229. She is registered with The Royal College of Occupational Therapists, and insured through them.

If you do not agree to the following terms & conditions you may wish to discontinue using this website, and or refrain from submitting your personal data to us and booking services.

Disclaimer

My services do not claim to ‘cure’ therefore are not guaranteed to have a positive effect. Further complementary therapy treatments may be required to have a desired effect, or additional medical intervention.

Whilst I am able to diagnose Lymphoedema, due to my qualification, I do not provide this service and will support you in accessing a diagnosis from your GP through provision of supporting letters.

It is best practice to make your own health care professional(s) aware of additional services you are attending and medical advice should always be sought by your GP, specialist consultant or whoever else is your regular health care provider.

Right to Refuse

I am proud of my qualifications, reputation and professional standards as an Occupational Therapist registered with the Health & Social Care Profession. If after consultation or if your circumstances have changed since filling in the consultation form, I will not carry out the treatment if I deem it unsafe for you.

PLEASE NOTE - If you do not disclose a health condition or contraindication to treatment and harm occurs, I cannot not be held responsible.

GDPR

Please read the Privacy Policy available on the website.

A thorough consultation form is required at the point of assessment. The information gathered is to ensure the treatment is appropriate for you. Data is stored securely and never passed on to other providers, including NHS services.

I run extremely confidential services and you can be confident that no information is shared, however if there are safeguarding concerns, I have a duty, as an Occupational Therapist, to report these.

Behaviour

I run a zero-tolerance business. I will not tolerate physical or verbal abuse from clients or partners. Appointments will be ended if I feel uncomfortable or threatened and I will not hesitate to contact the police if I feel appropriate to do so.
